Wholesale approval does not automatically make purchases tax-exempt. To request it, check the box above and complete the Certificate of Resale (CRT-61) fields, then sign electronically at the bottom. Submitting sends your completed certificate to the bakery to keep on file.

Prefer the official state form? Use the blank CRT-61 Certificate of Resale (PDF) and email a signed copy to deiyaabakery@gmail.com. Purchasers registered in another state use the same form with that state and registration number.

Your signed certificate must include:

Business name and address
Illinois retailer or reseller (IDOR) account number, or your state and registration number if out of state
A description of the products you buy for resale
Whether it is a blanket certificate (all purchases are for resale) or a single purchase
Your signature, printed name, title and date

The bakery must review your certificate before applying an exemption. You can apply and make taxable purchases without one.
